A review of mistakes and accidents at Quebec's hospitals, rehab institutions and long term care centres turned up 100 deaths in the six month period ending in March. 

There were more than 225-thousand problems logged, from falls to medication errors, some minor, some fatal. 

A report to be released today and obtained by La Presse shows more than half of the accidents and mistakes involved patients over the age of 75. 

The MUHC logged 17-hundred medication errors and 650 falls.  The CHUM reported 11-hundred medication mistakes and 930 falls.
